What are Baby Lotion (topical emollients)?

Emollients are substances that moisten and soften your skin.


Topical (for the skin) emollients are used to treat or prevent dry skin. Topical emollients are sometimes contained in products that also treat acne, chapped lips, diaper rash, cold sores, or other minor skin irritation.


There are many brands and forms of topical emollients available and not all are listed on this leaflet.


Topical emollients may also be used for purposes not listed in this medication guide.


What is the most important information I should know about Baby Lotion (topical emollients)?


You should not use a topical emollient if you are allergic to it. Topical emollients will not treat or prevent a skin infection.

Ask a doctor or pharmacist before using this medication if you have deep wounds or open sores, swelling, warmth, redness, oozing, bleeding, large areas of skin irritation, or any type of allergy.


What should I discuss with my healthcare provider before using Baby Lotion (topical emollients)?


You should not use a topical emollient if you are allergic to it. Topical emollients will not treat or prevent a skin infection.

Ask a doctor or pharmacist if it is safe for you to use this medicine if you have:



  • deep wounds or open sores;




  • swelling, warmth, redness, oozing, or bleeding;




  • large areas of skin irritation;




  • any type of allergy; or



  • if you are pregnant or breast-feeding.

How should I use Baby Lotion (topical emollients)?


Use exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended.


Clean the skin where you will apply the topical emollient. It may help to apply this product when your skin is wet or damp. Follow directions on the product label.


Shake the product container if recommended on the label.

Apply a small amount of topical emollient to the affected area and rub in gently.


If you are using a stick, pad, or soap form of topical emollient, follow directions for use on the product label.


Do not use this product over large area of skin. Do not apply a topical emollient to a deep puncture wound or severe burn without medical advice.

If your skin appears white or gray and feels soggy, you may be applying too much topical emollient or using it too often.


Some forms of topical emollient may be flammable and should not be used near high heat or open flame, or applied while you are smoking.

Store as directed away from moisture, heat, and light. Keep the bottle, tube, or other container tightly closed when not in use.


What happens if I miss a dose?


Since this product is used as needed, it does not have a daily dosing schedule. Seek medical advice if your condition does not improve after using a topical emollient.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

What should I avoid while taking Baby Lotion (topical emollients)?


Avoid getting topical emollients in your eyes, nose, or mouth. If this does happen, rinse with water. Avoid exposure to sunlight or tanning beds. Some topical emollients can make your skin more sensitive to sunlight or UV rays.

Baby Lotion (topical emollients) side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using the topical emollient and call your doctor if you have severe burning, stinging, redness, or irritation where the product was applied.

Less serious side effects are more likely, and you may have none at all.

What other drugs will affect Baby Lotion (topical emollients)?


It is not likely that other drugs you take orally or inject will have an effect on topically applied products. But many drugs can interact with each other. BetaTan Suspension


Pronunciation: BROME-fen-IR-a-meen/KAR-bay-ta-PEN-tane/FEN-il-EF-rin
Generic Name: Brompheniramine/Carbetapentane/Phenylephrine
Brand Name: Examples include V-Cof and Vazotan


BetaTan Suspension is used for:

Relieving symptoms of sinus congestion, pressure, runny nose, sneezing, and cough due to colds, upper respiratory infections, and allergies. It may also be used for other conditions as determined by your doctor.


BetaTan Suspension is an antihistamine, decongestant, and cough suppressant combination. The antihistamine works by blocking the action of histamine, which helps reduce symptoms such as watery eyes and sneezing. The decongestant promotes sinus and nasal drainage, which relieves congestion and pressure. The cough suppressant works in the brain to help decrease the cough reflex.


Do NOT use BetaTan Suspension if:


  • you are allergic to any ingredient in BetaTan Suspension

  • you have severe high blood pressure, severe heart blood vessel disease, rapid heartbeat, or severe heart problems

  • you take sodium oxybate (GHB) or you have taken furazolidone or a monoamine oxidase inhibitor (MAOI) (eg, phenelzine) within the last 14 days

Contact your doctor or health care provider right away if any of these apply to you.



Before using BetaTan Suspension:


Some medical conditions may interact with BetaTan Suspension. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:


  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you have a fast, slow, or irregular heartbeat

  • if you have a history of asthma; lung problems (eg, emphysema); adrenal gland problems (eg, adrenal gland tumor); heart problems; high blood pressure; diabetes; blood vessel problems; stroke; glaucoma; a blockage of your bladder, stomach, or intestines; ulcers; trouble urinating; an enlarged prostate or other prostate problems; seizures; phenylketonuria; or an overactive thyroid

  • if you are very drowsy, sedated, or are confined to a bed or chair

Some MEDICINES MAY INTERACT with BetaTan Suspension. Tell your health care provider if you are taking any other medicines, especially any of the following:


  • Beta-blockers (eg, propranolol), catechol-O-methyltransferase (COMT) inhibitors (eg, tolcapone), furazolidone, indomethacin, MAOIs (eg, phenelzine), sodium oxybate (GHB), or tricyclic antidepressants (eg, amitriptyline) because they may increase the risk of BetaTan Suspension's side effects

  • Digoxin or droxidopa because the risk of irregular heartbeat or heart attack may be increased

  • Bromocriptine or hydantoins (eg, phenytoin) because the risk of their side effects may be increased by BetaTan Suspension

  • Guanadrel, guanethidine, mecamylamine, methyldopa, or reserpine because their effectiveness may be decreased by BetaTan Suspension

This may not be a complete list of all interactions that may occur. Ask your health care provider if BetaTan Suspension may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.


How to use BetaTan Suspension:


Use BetaTan Suspension as directed by your doctor. Check the label on the medicine for exact dosing instructions.


  • Take BetaTan Suspension by mouth with or without food.

  • Shake well before each use.

  • Use a measuring device marked for medicine dosing. Ask your pharmacist for help if you are unsure of how to measure your dose.

  • If you miss a dose of BetaTan Suspension, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use BetaTan Suspension.



Important safety information:


  • BetaTan Suspension may cause drowsiness, dizziness, or blurred vision. These effects may be worse if you take it with alcohol or certain medicines. Use BetaTan Suspension with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.

  • Do not drink alcohol while you are using BetaTan Suspension.

  • Do not take diet or appetite control medicines while you use BetaTan Suspension unless your doctor tells you to.

  • BetaTan Suspension has brompheniramine and phenylephrine in it. Before you start any new medicine, check the label to see if it has brompheniramine and phenylephrine in it too. If it does or if you are not sure, check with your doctor or pharmacist.

  • Do NOT take more than the recommended dose or take BetaTan Suspension for longer than prescribed without checking with your doctor.

  • If your symptoms do not get better within 7 days or if they get worse, check with your doctor.

  • BetaTan Suspension may cause you to become sunburned more easily. Avoid the sun, sunlamps, or tanning booths until you know how you react to BetaTan Suspension. Use a sunscreen or wear protective clothing if you must be outside for more than a short time.

  • Some of these products contain phenylalanine. If you must have a diet that is low in phenylalanine, ask your pharmacist if it is in your product.

  • BetaTan Suspension may interfere with skin allergy tests. If you are scheduled for a skin test, talk to your doctor. You may need to stop taking BetaTan Suspension for a few days before the tests.

  • Tell your doctor or dentist that you take BetaTan Suspension before you receive any medical or dental care, emergency care, or surgery.

  • Use BetaTan Suspension with caution in the ELDERLY; they may be more sensitive to its effects.

  • Caution is advised when using BetaTan Suspension in CHILDREN; they may be more sensitive to its effects.

  • BetaTan Suspension should be used with extreme caution in CHILDREN younger than 2 years old; safety and effectiveness in these children have not been confirmed.

  • PREGNANCY and BREAST-FEEDING: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of using BetaTan Suspension while you are pregnant. It is not known if BetaTan Suspension is found in breast milk. Do not breast-feed while taking BetaTan Suspension.


Possible side effects of BetaTan Suspension:


All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:



Constipation; diarrhea; dizziness; drowsiness; excitability; headache; loss of appetite; nausea; nervousness or anxiety; trouble sleeping; upset stomach; vomiting; weakness.



Seek medical attention right away if any of these SEVERE side effects occur:

Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); difficulty urinating or inability to urinate; fast or irregular heartbeat; hallucinations; seizures; severe dizziness, lightheadedness, or headache; tremor; vision changes.

If OVERDOSE is suspected:


Contact 1-800-222-1222 (the American Association of Poison Control Centers), your local poison control center, or emergency room immediately. Symptoms may include blurred vision; confusion; hallucinations; seizures; severe dizziness, lightheadedness, or headache; severe drowsiness; unusually fast, slow, or irregular heartbeat; vomiting.


Proper storage of BetaTan Suspension:

Store BetaTan Suspension at room temperature, between 68 and 77 degrees F (20 and 25 degrees C). Store away from heat, moisture, and light. Do not store in the bathroom. Keep BetaTan Suspension out of the reach of children and away from pets.

Uses For Baridium


Phenazopyridine is used to relieve the pain, burning, and discomfort caused by infection or irritation of the urinary tract. It is not an antibiotic and will not cure the infection itself.


In the U.S., phenazopyridine is available without a prescription.


Before Using Baridium


In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For this medicine, the following should be considered:


Allergies


Tell your doctor if you have ever had any unusual or allergic reaction to this medicine or any other medicines. Also tell your health care professional if you have any other types of allergies, such as to foods, dyes, preservatives, or animals. For non-prescription products, read the label or package ingredients carefully.


Pediatric


Although there is no specific information comparing use of phenazopyridine in children with use in other age groups, it is not expected to cause different side effects or problems in children than it does in adults.


Geriatric


Many medicines have not been studied specifically in older people. Therefore, it may not be known whether they work exactly the same way they do in younger adults. Although there is no specific information comparing use of phenazopyridine in the elderly with use in other age groups, this medicine is not expected to cause different side effects or problems in older people than it does in younger adults.


Pregnancy








Pregnancy CategoryExplanation
All TrimestersBAnimal studies have revealed no evidence of harm to the fetus, however, there are no adequate studies in pregnant women OR animal studies have shown an adverse effect, but adequate studies in pregnant women have failed to demonstrate a risk to the fetus.

Breast Feeding


There are no adequate studies in women for determining infant risk when using this medication during breastfeeding. Weigh the potential benefits against the potential risks before taking this medication while breastfeeding.


Interactions with Medicines


Although certain medicines should not be used together at all, in other cases two different medicines may be used together even if an interaction might occur. In these cases, your doctor may want to change the dose, or other precautions may be necessary. Tell your healthcare professional if you are taking any other prescription or nonprescription (over-the-counter [OTC]) medicine.


Interactions with Food/Tobacco/Alcohol


Certain medicines should not be used at or around the time of eating food or eating certain types of food since interactions may occur. Using alcohol or tobacco with certain medicines may also cause interactions to occur. Discuss with your healthcare professional the use of your medicine with food, alcohol, or tobacco.


Other Medical Problems


The presence of other medical problems may affect the use of this medicine. Make sure you tell your doctor if you have any other medical problems, especially:


  • Glucose-6-phosphate dehydrogenase (G6PD) deficiency or

  • Hepatitis or

  • Kidney disease—The chance of side effects may be increased.

Proper Use of phenazopyridine

This section provides information on the proper use of a number of products that contain phenazopyridine. It may not be specific to Baridium. Please read with care.


This medicine is best taken with food or after eating a meal or a snack to lessen stomach upset.


Do not use any leftover medicine for future urinary tract problems without first checking with your doctor. An infection may require additional medicine.


Dosing


The dose of this medicine will be different for different patients. Follow your doctor's orders or the directions on the label. The following information includes only the average doses of this medicine. If your dose is different, do not change it unless your doctor tells you to do so.


The amount of medicine that you take depends on the strength of the medicine. Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine.


  • For oral dosage form (tablets):
    • For relieving pain, burning, and discomfort in the urinary tract:
      • Adults and teenagers—200 milligrams (mg) three times a day.

      • Children—The dose is based on body weight and must be determined by your doctor. The usual dose is 4 mg per kilogram (kg) (about 1.8 mg per pound) of body weight three times a day.



Missed Dose


If you miss a dose of this medicine, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.


Storage


Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light. Keep from freezing.


Keep out of the reach of children.


Do not keep outdated medicine or medicine no longer needed.


Precautions While Using Baridium


Check with your doctor if symptoms such as bloody urine, difficult or painful urination, frequent urge to urinate, or sudden decrease in the amount of urine appear or become worse while you are taking this medicine .


Phenazopyridine causes the urine to turn reddish orange . This is to be expected while you are using it. This effect is harmless and will go away after you stop taking the medicine. Also, the medicine may stain clothing.


For patients who wear soft contact lenses:


  • It is best not to wear soft contact lenses while being treated with this medicine. Phenazopyridine may cause discoloration or staining of contact lenses. It may not be possible to remove the stain.

For diabetic patients:


  • This medicine may cause false test results with urine sugar tests and urine ketone tests. If you have any questions about this, check with your health care professional, especially if your diabetes is not well controlled.

Before you have any medical tests, tell the person in charge that you are taking this medicine. The results of some tests may be affected by this medicine.


Baridium Side Effects


Along with its needed effects, a medicine may cause some unwanted effects. Although not all of these side effects may occur, if they do occur they may need medical attention.


Check with your doctor as soon as possible if any of the following side effects occur:


Rare
  • Blue or blue-purple color of skin

  • fever and confusion

  • shortness of breath, tightness in chest, wheezing, or troubled breathing

  • skin rash

  • sudden decrease in the amount of urine

  • swelling of face, fingers, feet, and/or lower legs

  • unusual tiredness or weakness

  • weight gain

  • yellow eyes or skin

Some side effects may occur that usually do not need medical attention. These side effects may go away during treatment as your body adjusts to the medicine. Also, your health care professional may be able to tell you about ways to prevent or reduce some of these side effects. Check with your health care professional if any of the following side effects continue or are bothersome or if you have any questions about them:


Less common or rare
  • Dizziness

  • headache

  • indigestion

  • itching of the skin

  • stomach cramps or pain

Other side effects not listed may also occur in some patients. If you notice any other effects, check with your healthcare professional.


Call your doctor for medical advice about side effects. You may report side effects to the FDA at 1-800-FDA-1088.

What is Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?

Brompheniramine is an antihistamine that reduces the effects of the natural chemical histamine in the body. Histamine can produce symptoms of sneezing, itching, watery eyes, and runny nose.


Dextromethorphan is a cough suppressant. It affects the signals in the brain that trigger cough reflex.


Pseudoephedrine is a decongestant that shrinks blood vessels in the nasal passages. Dilated blood vessels can cause nasal congestion (stuffy nose).


The combination of brompheniramine, dextromethorphan, and pseudoephedrine is used to treat runny or stuffy nose, sneezing, itching, watery eyes, cough, and sinus congestion caused by allergies, the common cold, or the flu.


This medicine will not treat a cough that is caused by smoking, asthma, or emphysema.

Brompheniramine, dextromethorphan, and pseudoephedrine may also be used for purposes not listed in this medication guide.


What is the most important information I should know about Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?


Do not give this medication to a child younger than 4 years old. Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children. You should not use this medication if you have severe constipation, a blockage in your stomach or intestines, or if you are unable to urinate. Do not use cough or cold medicine if you have untreated or uncontrolled diseases such as glaucoma, asthma or COPD, high blood pressure, heart disease, coronary artery disease, or overactive thyroid. Do not use cough or cold medicine if you have used an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate) in the last 14 days. A dangerous drug interaction could occur, leading to serious side effects.

What should I discuss with my healthcare provider before taking Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?


Do not use cough or cold medicine if you have used an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate) in the last 14 days. A dangerous drug interaction could occur, leading to serious side effects. You should not use this medication if you have severe constipation, a blockage in your stomach or intestines, or if you are unable to urinate. Do not use cough or cold medicine if you have untreated or uncontrolled diseases such as glaucoma, asthma or COPD, high blood pressure, heart disease, coronary artery disease, or overactive thyroid.

Ask a doctor or pharmacist if it is safe for you to take this medication if you have:



  • a blockage in your digestive tract (stomach or intestines), a colostomy or ileostomy;




  • diabetes;




  • liver or kidney disease;




  • epilepsy or other seizure disorder;




  • cough with mucus, or cough caused by emphysema or chronic bronchitis;




  • enlarged prostate or urination problems;




  • low blood pressure;




  • pheochromocytoma (an adrenal gland tumor); or




  • if you take potassium (Cytra, Epiklor, K-Lyte, K-Phos, Kaon, Klor-Con, Polycitra, Urocit-K).




FDA pregnancy category C. It is not known whether this medication will harm an unborn baby. Do not use cough or cold medicine without medical advice if you are pregnant. This medicine may pass into breast milk and may harm a nursing baby. Antihistamines and decongestants may also slow breast milk production. Do not use cough or cold medicine without medical advice if you are breast-feeding a baby.

How should I take Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?


Use exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended. Cough or cold medicine is usually taken only for a short time until your symptoms clear up.


Do not give this medication to a child younger than 4 years old. Always ask a doctor before giving a cough or cold medicine to a child. Death can occur from the misuse of cough and cold medicines in very young children.

Do not take for longer than 7 days in a row. Talk with your doctor if your symptoms do not improve after 7 days of treatment, or if you have a fever with a headache or skin rash.


Measure liquid medicine with a special dose-measuring spoon or medicine cup, not with a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one.


If you need surgery or medical tests, tell the surgeon or doctor ahead of time if you have taken a cough or cold medicine within the past few days. Store at room temperature away from moisture and heat. Do not freeze.

What happens if I miss a dose?


Since cough or cold medicine is taken when needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.


What happens if I overdose?


Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

Overdose symptoms may include severe forms of some of the side effects listed in this medication guide.


What should I avoid while taking Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?


This medicine may cause blurred vision and may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert and able to see clearly. Drinking alcohol can increase certain side effects of this medication. Ask a doctor or pharmacist before using any other cold, allergy, cough, or sleep medicine. Antihistamines, cough suppressants, and decongestants are contained in many combination medicines. Taking certain products together can cause you to get too much of a certain drug. Check the label to see if a medicine contains an antihistamine, cough suppressant, or decongestant.

Avoid taking this medication if you also take diet pills, caffeine pills, or other stimulants (such as ADHD medications). Taking a stimulant together with a decongestant can increase your risk of unpleasant side effects.


Avoid becoming overheated or dehydrated during exercise and in hot weather. This medication can decrease sweating and you may be more prone to heat stroke.

Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine) side effects


Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using this medicine and call your doctor at once if you have a serious side effect such as:

  • fast, slow, or uneven heart rate;




  • severe headache, mood changes, hallucinations;




  • severe dizziness or anxiety, feeling like you might pass out;




  • tremor, seizure (convulsions);




  • easy bruising or bleeding, unusual weakness;




  • fever;




  • urinating less than usual or not at all;




  • feeling short of breath; or




  • dangerously high blood pressure (severe headache, blurred vision, buzzing in your ears, anxiety, confusion, chest pain, uneven heartbeats, seizure).



Less serious side effects may include:



  • mild headache;




  • mild dizziness, drowsiness;




  • dry mouth, nose, or throat;




  • nausea, diarrhea, constipation, upset stomach;




  • feeling nervous, restless, or irritable;




  • blurred vision; or




  • sleep problems (insomnia).

What other drugs will affect Bromplex DM (brompheniramine, dextromethorphan, and pseudoephedrine)?


Ask a doctor or pharmacist before using this medicine if you regularly use other medicines that make you sleepy (such as narcotic pain medication, sedatives, sleeping pills, muscle relaxers, and medicine for seizures, depression or anxiety). They can add to sleepiness caused by brompheniramine or dextromethorphan.


Ask a doctor or pharmacist if it is safe for you to take this medication if you are also using any of the following drugs:



  • atropine (Atreza, Sal-Tropine);




  • benztropine (Cogentin);




  • topiramate (Topamax);




  • zonisamide (Zonegran);




  • an antidepressant such as citalopram (Celexa), duloxetine (Cymbalta), escitalopram (Lexapro), fluoxetine (Prozac, Sarafem, Symbyax), paroxetine (Paxil, Pexeva), sertraline (Zoloft), venlafaxine (Effexor), and others;




  • anti-nausea medications such as belladonna (Donnatal), dimenhydrinate (Dramamine), droperidol (Inapsine), methscopolamine (Pamine), or scopolamine (Transderm Scop);




  • bladder or urinary medications such as darifenacin (Enablex), flavoxate (Urispas), oxybutynin (Ditropan, Oxytrol), solifenacin (Vesicare), tolterodine (Detrol), or Urogesic Blue;




  • bronchodilators such as ipratropium (Atrovent) or tiotropium (Spiriva);




  • irritable bowel medications such as dicyclomine (Bentyl), hyoscyamine (Hyomax), or propantheline (Pro Banthine); or




  • ulcer medicine such as glycopyrrolate (Robinul) or mepenzolate (Cantil).

What is BexxarTherapeutic (tositumomab)?

Tositumomab is a monoclonal antibody that is linked with radioactive iodine I-131. Monoclonal antibodies are made to target and destroy only certain cells in the body. This may help to protect healthy cells from damage.


Tositumomab is used to treat certain forms of non-Hodgkin's lymphoma. It is usually given after other medications have been tried without successful treatment.


Tositumomab may also be used for other purposes not listed in this medication guide.


What is the most important information I should know about BexxarTherapeutic (tositumomab)?


You should not receive this medication if you are allergic to mouse proteins, or if you have ever had an allergic reaction to a monoclonal antibody.

Before you receive tositumomab, tell your doctor if you have kidney disease or severe bone marrow suppression.


Tositumomab can cause damage to the thyroid gland of an unborn baby if the mother receives this medication during pregnancy. You should not receive this medication if you are pregnant.

Tositumomab can be harmful to an unborn baby whether the father or the mother receives this medication at the time of conception. Use effective birth control to prevent pregnancy during your treatment, whether you are a man or a woman. Continue using birth control for at least 12 months after you have received tositumomab.


Do not receive a "live" vaccine just after you receive tositumomab. After you are treated with tositumomab, your body will retain radioactive material for several days. During this time you will be giving off a small amount of radiation to others around you. Your caregivers should give you instructions about how to avoid exposing other people to radiation until this effect wears off. Follow these instructions carefully. Take all medications your doctor prescribes to help protect your thyroid gland when you receive tositumomab. You may need to have blood tests to check your thyroid function at regular intervals for the rest of your life. Visit your doctor regularly.

What should I discuss with my health care provider before I receive BexxarTherapeutic (tositumomab)?


You should not receive this medication if you are allergic to mouse proteins, or if you have ever had an allergic reaction to a monoclonal antibody. FDA pregnancy category X. Tositumomab can cause damage to the thyroid gland of an unborn baby if the mother receives this medication during pregnancy. You should not receive this medication if you are pregnant.

Tositumomab can be harmful to an unborn baby whether the father or the mother receives this medication at the time of conception. Some people treated with tositumomab later developed leukemia or other cancers. However, it has not been determined whether this medication actually increases the risk of causing other cancers. Talk with your doctor about your individual risk.


How is tositumomab given?


Before and during your tositumomab treatment, you will be given other medications to protect your thyroid gland and to help prevent allergic reactions. You may need to keep taking some of these medications for up to 2 weeks after your treatment.

The tositumomab treatment is usually given in two steps over a span of 7 days. In the first treatment step, you will receive the two injections just before you undergo an x-ray scan of your entire body. This scan will show your doctor how well the radioactive ingredients of the tositumomab injections are circulating throughout your body. You may receive at least 2 more scans over the next 6 or 7 days.


Based on the results of your scans, your doctor will determine whether or not to give the second step of tositumomab treatment, and how large your dose should be.


In step 2 of the treatment, you will again receive the two injections one at a time. These injections are considered your therapeutic dose. The 2-step tositumomab treatment is usually given only once, so you are not likely to receive a second course of treatment with this medication.


Wash your hands often to prevent infections. After you are treated with tositumomab, your body will retain radioactive material for several days. During this time you will be giving off a small amount of radiation to others around you. Your caregivers should give you instructions about how to avoid exposing other people to radiation until this effect wears off.

Tositumomab can increase your risk of developing hypothyroidism (underactive thyroid). Hypothyroidism can be treated with daily thyroid replacement medication.


Take all medications your doctor prescribes to help protect your thyroid gland when you receive tositumomab. You may need to have blood tests to check your thyroid function at regular intervals for the rest of your life. Visit your doctor regularly.

Tositumomab can cause you to have unusual results with certain medical tests. You may also have an allergic reaction to diagnostic tests or treatments using mouse proteins or monoclonal antibodies. Tell any doctor who treats you that you have been treated with tositumomab.


What happens if I miss a dose?


Since tositumomab is given in a controlled setting by a healthcare professional, it is not likely that you will miss a dose.


What happens if I overdose?


An overdose of this medication is very unlikely to occur.


What should I avoid after receiving BexxarTherapeutic (tositumomab)?


Do not receive a "live" vaccine just after you receive tositumomab. Ask your doctor how long after your treatment you should wait before receiving any vaccines.

Avoid sports or activities that may increase your risk of bleeding or injury. Use a soft toothbrush to keep your gums from bleeding. Take extra care not to cut yourself while shaving.


Avoid handling any of your body fluids without wearing latex rubber gloves. If another person is handling your fluids (vomit, stools, or urine), they should wear gloves, eye protection, and a mask to cover the nose and mouth.

When cleaning any spills of bodily fluid, use only disposable cleaning cloths that can be flushed down a toilet. Ask your doctor or health department how to dispose of any bodily fluid spills that cannot be flushed down a toilet.


To protect others from exposure to the radioactive matter in your body, try to stay at least 6 feet away from other people, especially children or pregnant women. Follow these steps for several days after your tositumomab treatment ends:



  • Avoid crowds and public places.




  • Avoid traveling on long trips.




  • Do not share a bed or bathroom with another person.




  • Sit on the toilet while urinating and flush 3 times with the lid down after use.




  • Always wash your hands after using the bathroom.




  • Do not share a towel, wash cloth, or toothbrush with another person.




  • Do not share drinking glasses, plates, or silverware.




  • Wait at least 1 week before washing any of the clothing and bed or bath linens you have used during the week after your treatment. Keep these items separate from the laundry of other people in your home.




  • Wash your clothing and other items separately from other laundry in your home.



BexxarTherapeutic (tositumomab) side effects


Some people receiving tositumomab have had a reaction to the infusion (when the medicine is injected into the vein). Tell your caregivers right away if you feel dizzy, nauseated, light-headed, sweaty, or short of breath, or if you have fever or chills during the injection. Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have a serious side effect such as:



  • pale skin, easy bruising or bleeding, unusual weakness;




  • black, bloody, or tarry stools;




  • blood in your urine;




  • fever, chills, sore throat, body aches, flu symptoms;




  • cough with yellow or green mucus; or




  • stabbing chest pain, feeling short of breath.



Less serious side effects may include:



  • decreased energy;




  • weight gain;




  • headache;




  • joint or muscle pain;




  • feeling weak or irritable;




  • increased thirst and hot, dry skin;




  • nausea, vomiting, diarrhea, stomach pain;




  • loss of appetite;




  • mild itching or skin rash;




  • runny or stuffy nose; or




  • pain, itching, swelling, or redness around your IV needle.

What is niacin?

Niacin, also called nicotinic acid, is a B vitamin (vitamin B3). It occurs naturally in plants and animals, and is also added to many foods as a vitamin supplement. Niacin is also present in many multiple vitamins and nutritional supplements.


Niacin is used to treat and prevent a lack of natural niacin in the body, and to lower cholesterol and triglycerides (types of fat) in the blood. It is also used to lower the risk of heart attack in people with high cholesterol who have already had a heart attack. Niacin is sometimes used to treat coronary artery disease (also called atherosclerosis).


Niacin may also be used for purposes other than those listed in this medication guide.


What is the most important information I should know about niacin?


Do not take this medication if you are allergic to niacin, or if you have severe liver disease, a stomach ulcer, or active bleeding.

Niacin can cause certain side effects, such as flushing (warmth, itching, redness, or tingly feeling under your skin). These effects can be made worse if you drink alcohol or hot beverages shortly after you take niacin. These effects should disappear over time as you keep taking the medication.


Avoid getting up too fast from a sitting or lying position, or you may feel dizzy. Get up slowly and steady yourself to prevent a fall.


Avoid taking colestipol (Colestid) or cholestyramine (Locholest, Prevalite, Questran) at the same time you take niacin. If you take either of these other medications, take them at least 4 to 6 hours before or after you take niacin.


Niacin is only part of a complete program of treatment that may also include diet, exercise, weight control, and other medications. Follow your diet, medication, and exercise routines very closely.


What should I discuss with my healthcare provider before taking niacin ?


Do not take this medication if you are allergic to niacin, or if you have severe liver disease, a stomach ulcer, or active bleeding.

Before taking niacin, tell your doctor if you are allergic to any drugs, or if you have:


  • liver or kidney disease;


  • heart disease or uncontrolled angina (chest pain);

  • a muscle disorder such as myasthenia gravis.



If you have any of these conditions, you may not be able to use niacin, or you may need dosage adjustments or special tests during treatment.


FDA pregnancy category C. Niacin may be harmful to an unborn baby when the medication is taken at doses to treat high cholesterol or other conditions. Tell your doctor if you are pregnant or plan to become pregnant during treatment. Niacin can pass into breast milk and may harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I take niacin ?


Take this medication exactly as it was prescribed for you. Do not take the medication in larger amounts, or take it for longer than recommended by your doctor. Follow the directions on your prescription label.


Niacin is sometimes taken at bedtime with a low-fat snack. Follow your doctor's instructions.


Niacin can cause certain side effects, such as flushing (warmth, itching, redness, or tingly feeling under your skin). These effects can be made worse if you drink alcohol or hot beverages shortly after you take niacin. These effects should disappear over time as you keep taking the medication.


Take niacin with a full glass of cold or cool water. Taking the medication with a hot drink may increase your risk of side effects such as flushing. Do not crush, chew, break, or open an extended-release tablet or capsule. Swallow the pill whole. It is specially made to release medicine slowly in the body. Breaking or opening the pill would cause too much of the drug to be released at one time. Niacin extended-release tablets and capsules contain higher strengths of the medicine than the regular niacin tablets. Take only the dose that is correct for the type of niacin tablet or capsule you are using.

Niacin can cause you to have unusual results with certain medical tests (urine tests). Tell any doctor who treats you that you are using niacin.


If you stop taking niacin for any length of time, talk with your doctor before starting the medication again. You may need to restart the medication at a lower dose.


Niacin is only part of a complete program of treatment that may also include diet, exercise, weight control, and other medications. Follow your diet, medication, and exercise routines very closely.


To be sure this medication is not causing harmful effects, your blood will need to be tested on a regular basis. Your kidney or liver function may also need to be tested. Do not miss any scheduled appointments.


Store niacin at room temperature away from moisture and heat.

What happens if I miss a dose?


Take the missed dose as soon as you remember. Be sure to take the missed dose with food if you normally take your niacin dose with a meal or snack.


If it is almost time for your next dose, skip the missed dose and take the medicine at your next regularly scheduled time. Do not take extra medicine to make up the missed dose.


What happens if I overdose?

What should I avoid while taking niacin ?

Avoid drinking alcohol while taking niacin. Alcohol may increase your risk of liver damage, and can also worsen the flushing effects of niacin.

Avoid taking colestipol (Colestid) or cholestyramine (Locholest, Prevalite, Questran) at the same time you take niacin. If you take either of these other medications, take them at least 4 to 6 hours before or after you take niacin.


Avoid getting up too fast from a sitting or lying position, or you may feel dizzy. Get up slowly and steady yourself to prevent a fall.


Niacin side effects

If you are diabetic, tell your doctor about any changes in your blood sugar levels.

Less serious side effects of niacin include:



  • mild dizziness;




  • warmth, redness, or tingly feeling under your skin;




  • itching, dry skin;




  • sweating or chills;




  • nausea, diarrhea, belching, gas;




  • muscle pain, leg cramps; or




  • sleep problems (insomnia).

What other drugs will affect niacin ?


Tell your doctor about all other cholesterol-lowering drugs you are taking with niacin, especially atorvastatin (Lipitor), fluvastatin (Lescol), lovastatin (Mevacor), pravastatin (Pravachol), or simvastatin (Zocor).


Before taking niacin, tell your doctor if you are also using any of the following drugs:



  • a blood thinner such as warfarin (Coumadin);




  • multivitamins or mineral supplements that contain niacin;




  • blood pressure or heart medications such as amlodipine (Norvasc), diltiazem (Tiazac, Cartia, Cardizem), felodipine (Plendil), nicardipine (Cardene), nifedipine (Procardia, Adalat), nimodipine (Nimotop), nisoldipine (Sular), or verapamil (Calan, Covera, Isoptin, Verelan); or




  • heart medications such as doxazosin (Cardura), isosorbide (Dilatrate, Imdur, Isordil, Monoket, Sorbitrate), nitroglycerin (Nitro-Bid, Nitro-Dur, Nitrostat), prazosin (Minipress), or terazosin (Hytrin).



This list is not complete and there may be other drugs that can interact with niacin. Tell your doctor about all the prescription and over-the-counter medications you use. Bystolic Description


The chemical name for the active ingredient in Bystolic (nebivolol) tablets is (1RS,1'RS)-1,1'-[(2RS,2'SR)-bis(6-fluoro-3,4-dihydro-2H-1-benzopyran-2-yl)]- 2,2'-iminodiethanol hydrochloride. Nebivolol is a racemate composed of d-Nebivolol and l-Nebivolol with the stereochemical designations of [SRRR]-nebivolol and [RSSS]-nebivolol, respectively. Nebivolol's molecular formula is (C22H25F2NO4•HCl) with the following structural formula:



Nebivolol hydrochloride is a white to almost white powder that is soluble in methanol, dimethylsulfoxide, and N,N-dimethylformamide, sparingly soluble in ethanol, propylene glycol, and polyethylene glycol, and very slightly soluble in hexane, dichloromethane, and methylbenzene.


Bystolic as tablets for oral administration contains nebivolol hydrochloride equivalent to 2.5, 5, 10, and 20 mg of nebivolol base. In addition, Bystolic contains the following inactive ingredients: colloidal silicon dioxide, croscarmellose sodium, D&C Red #27 Lake, FD&C Blue #2 Lake, FD&C Yellow #6 Lake, hypromellose, lactose monohydrate, magnesium stearate, microcrystalline cellulose, pregelatinized starch, polysorbate 80, and sodium lauryl sulfate.

General


Nebivolol is a β-adrenergic receptor blocking agent. In extensive metabolizers (most of the population) and at doses less than or equal to 10 mg, nebivolol is preferentially β1 selective. In poor metabolizers and at higher doses, nebivolol inhibits both β1 and β2 - adrenergic receptors. Nebivolol lacks intrinsic sympathomimetic and membrane stabilizing activity at therapeutically relevant concentrations. At clinically relevant doses, Bystolic does not demonstrate α1-adrenergic receptor blockade activity. Various metabolites, including glucuronides, contribute to β-blocking activity.



Pharmacodynamics


The mechanism of action of the antihypertensive response of Bystolic has not been definitively established. Possible factors that may be involved include: (1) decreased heart rate, (2) decreased myocardial contractility, (3) diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers, (4) suppression of renin activity and (5) vasodilation and decreased peripheral vascular resistance.



Pharmacokinetics


Nebivolol is metabolized by a number of routes, including glucuronidation and hydroxylation by CYP2D6. The active isomer (d-nebivolol) has an effective half-life of about 12 hours in CYP2D6 extensive metabolizers (most people), and 19 hours in poor metabolizers and exposure to d-nebivolol is substantially increased in poor metabolizers. This has less importance than usual, however, because the metabolites, including the hydroxyl metabolite and glucuronides (the predominant circulating metabolites), contribute to β-blocking activity.


Plasma levels of d–nebivolol increase in proportion to dose in EMs and PMs for doses up to 20mg. Exposure to l-nebivolol is higher than to d-nebivolol but l-nebivolol contributes little to the drug's activity as d-nebivolol's beta receptor affinity is > 1000-fold higher than l-nebivolol. For the same dose, PMs attain a 5-fold higher Cmax and 10-fold higher AUC of d-nebivolol than do EMs. d-Nebivolol accumulates about 1.5-fold with repeated once-daily dosing in EMs.


Absorption and Distribution

Absorption of Bystolic is similar to an oral solution. The absolute bioavailability has not been determined.


Mean peak plasma nebivolol concentrations occur approximately 1.5 to 4 hours post-dosing in EMs and PMs.


Food does not alter the pharmacokinetics of nebivolol. Under fed conditions, nebivolol glucuronides are slightly reduced. Bystolic may be administered without regard to meals.


The in vitro human plasma protein binding of nebivolol is approximately 98%, mostly to albumin, and is independent of nebivolol concentrations.


Metabolism and Excretion

Nebivolol is predominantly metabolized via direct glucuronidation of parent and to a lesser extent via N-dealkylation and oxidation via cytochrome P450 2D6. Its stereospecific metabolites contribute to the pharmacologic activity (see Drug Interactions).


After a single oral administration of 14C-nebivolol, 38% of the dose was recovered in urine and 44% in feces for EMs and 67% in urine and 13% in feces for PMs. Essentially all nebivolol was excreted as multiple oxidative metabolites or their corresponding glucuronide conjugates.



Drug-Interactions


Drugs that inhibit CYP2D6 can be expected to increase plasma levels of nebivolol. When Bystolic is co-administered with an inhibitor or an inducer of this enzyme, patients should be closely monitored and the nebivolol dose adjusted according to blood pressure response. In vitro studies have demonstrated that at therapeutically relevant concentrations, d- and l-nebivolol do not inhibit any cytochrome P450 pathways.



Digoxin: Concomitant administration of Bystolic (10 mg once daily) and digoxin (0.25 mg once daily) for 10 days in 14 healthy adult individuals resulted in no significant changes in the pharmacokinetics of digoxin or nebivolol (see PRECAUTION, Drug Interactions).



Warfarin: Administration of Bystolic (10 mg once daily for 10 days) led to no significant changes in the pharmacokinetics of nebivolol or R- or S-warfarin following a single 10 mg dose of warfarin. Similarly, nebivolol has no significant effects on the anticoagulant activity of warfarin, as assessed by Prothrombin time and INR profiles from 0 to 144 hours after a single 10 mg warfarin dose in 12 healthy adult volunteers.



Diuretics: No pharmacokinetic interactions were observed in healthy adults between nebivolol (10 mg daily for 10 days) and furosemide (40 mg single dose), hydrochlorothiazide (25 mg once daily for 10 days), or spironolactone (25 mg once daily for 10 days).



Ramipril: Concomitant administration of Bystolic (10 mg once daily) and ramipril (5 mg once daily) for 10 days in 15 healthy adult volunteers produced no pharmacokinetic interactions.



Losartan: Concomitant administration of Bystolic (10 mg single dose) and losartan (50 mg single dose) in 20 healthy adult volunteers did not result in pharmacokinetic interactions.



Fluoxetine: Fluoxetine, a CYP2D6 inhibitor, administered at 20 mg per day for 21 days prior to a single 10 mg dose of nebivolol to 10 healthy adults, led to an 8-fold increase in the AUC and 3-fold increase in Cmax for d-nebivolol (see PRECAUTIONS, Drug Interactions).



Histamine-2 Receptor Antagonists: The pharmacokinetics of nebivolol (5 mg single dose) were not affected by the co-administration of ranitidine (150 mg twice daily). Cimetidine (400 mg twice daily) causes a 23% increase in the plasma levels of d-nebivolol.



Charcoal: The pharmacokinetics of nebivolol (10 mg single dose) were not affected by repeated co-administration (4, 8, 12, 16, 22, 28, 36, and 48 hours after nebivolol administration) of activated charcoal (Actidose-Aqua®).



Sildenafil: The co-administration of nebivolol and sildenafil decreased AUC and Cmax of sildenafil by 21 and 23% respectively. The effect on the Cmax and AUC for d-nebivolol was also small (< 20%). The effect on vital signs (e.g., pulse and blood pressure) was approximately the sum of the effects of sildenafil and nebivolol.



Other Concomitant Medications: Utilizing population pharmacokinetic analyses, derived from hypertensive patients, the following drugs were observed not to have an effect on the pharmacokinetics of nebivolol: acetaminophen, acetylsalicylic acid, atorvastatin, esomeprazole, ibuprofen, levothyroxine sodium, metformin, sildenafil, simvastatin, or tocopherol.



Protein Binding: No meaningful changes in the extent of in vitro binding of nebivolol to human plasma proteins were noted in the presence of high concentrations of diazepam, digoxin, diphenylhydantoin, enalapril, hydrochlorothiazide, imipramine, indomethacin, propranolol, sulfamethazine, tolbutamide, or warfarin. Additionally, nebivolol did not significantly alter the protein binding of the following drugs: diazepam, digoxin, diphenylhydantoin, hydrochlorothiazide, imipramine, or warfarin at their therapeutic concentrations.



Special Populations



Renal Disease: The apparent clearance of nebivolol was unchanged following a single 5 mg dose of Bystolic in patients with mild renal impairment (ClCr 50 to 80 mL/min, n=7), and it was reduced negligibly in patients with moderate (ClCr 30 to 50 mL/min, n=9), but by 53% in patients with severe renal impairment (ClCr <30 mL/min, n=5). The dose of Bystolic should be adjusted in patients with severe renal impairment. Bystolic should be used with caution in patients receiving dialysis, since no formal studies have been conducted in this population (see DOSAGE AND ADMINISTRATION).



Hepatic Disease: d-Nebivolol peak plasma concentration increased 3-fold, exposure (AUC) increased 10-fold, and the apparent clearance decreased by 86% in patients with moderate hepatic impairment (Child-Pugh Class B). The starting dose should be reduced in patients with moderate hepatic impairment. No formal studies have been performed in patients with severe hepatic impairment and nebivolol should be contraindicated for these patients (see DOSAGE AND ADMINISTRATION).



Clinical Studies


The antihypertensive effectiveness of Bystolic as monotherapy has been demonstrated in three randomized, double-blind, multi-center, placebo-controlled trials at doses ranging from 1.25 to 40 mg for 12 weeks (Studies 1, 2, and 3). A fourth placebo-controlled trial demonstrated additional antihypertensive effects of Bystolic at doses ranging from 5 to 20 mg when administered concomitantly with up to two other antihypertensive agents (ACE inhibitors, angiotensin II receptor antagonists, and thiazide diuretics) in patients with inadequate blood pressure control.


The three monotherapy trials included a total of 2016 patients (1811 Bystolic, 205 placebo) with mild to moderate hypertension who had baseline diastolic blood pressures (DBP) of 95 to 109 mmHg. Patients received either Bystolic or placebo once daily for twelve weeks. Two of these monotherapy trials (Studies 1 and 2) studied 1716 patients in the general hypertensive population with a mean age of 54 years, 55% males, 26% non-Caucasians, 7% diabetics and 6% genotyped as PMs. The third monotherapy trial (Study 3) studied 300 Black patients with a mean age of 51 years, 45% males, 14% diabetics, and 3% as PMs.


Placebo-subtracted blood pressure reductions by dose for each study are presented in Table 1. Most studies showed increasing response to doses above 5 mg.

Study 4 enrolled 669 patients with a mean age of 54 years, 55% males, 54% Caucasians, 29% Blacks, 15% Hispanics, 1% Asians, 14% diabetics, and 5% PMs. Bystolic, 5 mg to 20 mg, administered once daily concomitantly with stable doses of up to two other antihypertensive agents (ACE inhibitors, angiotensin II receptor antagonists, and thiazide diuretics) resulted in significant additional antihypertensive effects over placebo compared to baseline blood pressure.


Effectiveness was similar in subgroups analyzed by age and sex. Effectiveness was established in Blacks, but as monotherapy the magnitude of effect was somewhat less than in Caucasians.


The blood pressure lowering effect of Bystolic was seen within two weeks of treatment and was maintained over the 24-hour dosing interval.



Indications and Usage for Bystolic


Bystolic is indicated for the treatment of hypertension. Bystolic may be used alone or in combination with other antihypertensive agents.



Contraindications


Bystolic is contraindicated in patients with severe bradycardia, heart block greater than first degree, cardiogenic shock, decompensated cardiac failure, sick sinus syndrome (unless a permanent pacemaker is in place), or severe hepatic impairment (Child-Pugh >B), and in patients who are hypersensitive to any component of this product.



Warnings



Abrupt Cessation of Therapy


Patients with coronary artery disease treated with Bystolic should be advised against abrupt discontinuation of therapy. Severe exacerbation of angina and the occurrence of myocardial infarction and ventricular arrhythmias have been reported in patients with coronary artery disease following the abrupt discontinuation of therapy with β-blockers. Myocardial infarction and ventricular arrhythmias may occur with or without preceding exacerbation of the angina pectoris. Even patients without overt coronary artery disease should be cautioned against interruption or abrupt discontinuation of therapy. As with other β-blockers, when discontinuation of Bystolic is planned, patients should be carefully observed and advised to minimize physical activity. Bystolic should be tapered over 1 to 2 weeks when possible. If the angina worsens or acute coronary insufficiency develops, it is recommended that Bystolic be promptly reinstituted, at least temporarily.



Cardiac Failure


Sympathetic stimulation is a vital component supporting circulatory function in the setting of congestive heart failure, and β-blockade may result in further depression of myocardial contractility and precipitate more severe failure. In patients who have compensated congestive heart failure, Bystolic should be administered cautiously. If heart failure worsens, discontinuation of Bystolic should be considered.



Angina and Acute Myocardial Infarction


Bystolic was not studied in patients with angina pectoris or who had a recent MI.



Bronchospastic Diseases


In general, patients with bronchospastic diseases should not receive β-blockers.



Anesthesia and Major Surgery


If Bystolic is to be continued perioperatively, patients should be closely monitored when anesthetic agents which depress myocardial function, such as ether, cyclopropane, and trichloroethylene, are used. If β-blocking therapy is withdrawn prior to major surgery, the impaired ability of the heart to respond to reflex adrenergic stimuli may augment the risks of general anesthesia and surgical procedures.


The β-blocking effects of Bystolic can be reversed by β-agonists, e.g., dobutamine or isoproterenol. However, such patients may be subject to protracted severe hypotension. Additionally, difficulty in restarting and maintaining the heartbeat has been reported with β-blockers.



Diabetes and Hypoglycemia


β-blockers may mask some of the manifestations of hypoglycemia, particularly tachycardia. Nonselective β-blockers may potentiate insulin-induced hypoglycemia and delay recovery of serum glucose levels. It is not known whether nebivolol has these effects. Patients subject to spontaneous hypoglycemia, or diabetic patients receiving insulin or oral hypoglycemic agents, should be advised about these possibilities and nebivolol should be used with caution.



Thyrotoxicosis


β-blockers may mask clinical signs of hyperthyroidism, such as tachycardia. Abrupt withdrawal of β-blockers may be followed by an exacerbation of the symptoms of hyperthyroidism or may precipitate a thyroid storm.



Peripheral Vascular Disease


β-blockers can precipitate or aggravate symptoms of arterial insufficiency in patients with peripheral vascular disease. Caution should be exercised in these patients.



Non-dihydropyridine Calcium Channel Blockers


Because of significant negative inotropic and chronotropic effects in patients treated with β-blockers and calcium channel blockers of the verapamil and diltiazem type, caution should be used in patients treated concomitantly with these agents and ECG and blood pressure should be monitored.



Precautions



Use with CYP2D6 Inhibitors


Nebivolol exposure increases with inhibition of CYP2D6 (see Drug Interactions). The dose of Bystolic may need to be reduced.



Impaired Renal Function


Bystolic should be used with caution in patients with severe renal impairment because of decreased renal clearance. Bystolic has not been studied in patients receiving dialysis.



Impaired Hepatic Function


Bystolic should be used with caution in patients with moderate hepatic impairment because of decreased metabolism. Since Bystolic has not been studied in patients with severe hepatic impairment, Bystolic is contraindicated in this population (see CLINICAL PHARMACOLOGY, Special Populations and DOSAGE AND ADMINISTRATION).



Risk of Anaphylactic Reactions


While taking β-blockers, patients with a history of severe anaphylactic reactions to a variety of allergens may be more reactive to repeated challenge either accidental, diagnostic, or therapeutic. Such patients may be unresponsive to the usual doses of epinephrine used to treat allergic reactions.


In patients with known or suspected pheochromocytoma, an α-blocker should be initiated prior to the use of any β-blocker.



Information for Patients


Patients should be advised to take Bystolic regularly and continuously, as directed. Bystolic can be taken with or without food. If a dose is missed, the patient should take the next scheduled dose only (without doubling it). Patients should not interrupt or discontinue Bystolic without consulting the physician.


Patients should know how they react to this medicine before they operate automobiles, use machinery, or engage in other tasks requiring alertness.


Patients should be advised to consult a physician if any difficulty in breathing occurs, or if they develop signs or symptoms of worsening congestive heart failure such as weight gain or increasing shortness of breath, or excessive bradycardia.


Patients subject to spontaneous hypoglycemia, or diabetic patients receiving insulin or oral hypoglycemic agents, should be cautioned that β-blockers may mask some of the manifestations of hypoglycemia, particularly tachycardia. Nebivolol should be used with caution in these patients.



Drug Interactions


Bystolic should be used with care when myocardial depressants or inhibitors of AV conduction, such as certain calcium antagonists (particularly of the phenylalkylamine [verapamil] and benzothiazepine [diltiazem] classes), or antiarrhythmic agents, such as disopyramide, are used concurrently. Both digitalis glycosides and β-blockers slow atrioventricular conduction and decrease heart rate. Concomitant use can increase the risk of bradycardia.


Bystolic should not be combined with other β-blockers. Patients receiving catecholamine-depleting drugs, such as reserpine or guanethidine, should be closely monitored, because the added β-blocking action of Bystolic may produce excessive reduction of sympathetic activity. In patients who are receiving Bystolic and clonidine, Bystolic should be discontinued for several days before the gradual tapering of clonidine.



CYP2D6 Inhibitors: Use caution when Bystolic is co-administered with CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.) (see CLINICAL PHARMACOLOGY, Drug Interactions).



Carcinogenesis, Mutagenesis, Impairment of Fertility


In a two-year study of nebivolol in mice, a statistically significant increase in the incidence of testicular Leydig cell hyperplasia and adenomas was observed at 40 mg/kg/day (5 times the maximally recommended human dose of 40 mg on a mg/m2 basis). Similar findings were not reported in mice administered doses equal to approximately 0.3 or 1.2 times the maximum recommended human dose. No evidence of a tumorigenic effect was observed in a 24-month study in Wistar rats receiving doses of nebivolol 2.5, 10 and 40 mg/kg/day (equivalent to 0.6, 2.4, and 10 times the maximally recommended human dose). Co-administration of dihydrotestosterone reduced blood LH levels and prevented the Leydig cell hyperplasia, consistent with an indirect LH-mediated effect of nebivolol in mice and not thought to be clinically relevant in man.


A randomized, double-blind, placebo- and active-controlled, parallel-group study in healthy male volunteers was conducted to determine the effects of nebivolol on adrenal function, luteinizing hormone, and testosterone levels. This study demonstrated that 6 weeks of daily dosing with 10 mg of nebivolol had no significant effect on ACTH-stimulated mean serum cortisol AUC0-120 min, serum LH, or serum total testosterone.


Effects on spermatogenesis were seen in male rats and mice at ≥40 mg/kg/day (10 and 5 times the MRHD, respectively). For rats the effects on spermatogenesis were not reversed and may have worsened during a four week recovery period. The effects of nebivolol on sperm in mice, however, were partially reversible.


Mutagenesis: Nebivolol was not genotoxic when tested in a battery of assays (Ames, in vitro mouse lymphoma TK+/-, in vitro human peripheral lymphocyte chromosome aberration, in vivo Drosophila melanogaster sex-linked recessive lethal, and in vivo mouse bone marrow micronucleus tests).



Pregnancy: Teratogenic Effects. Pregnancy Category C:


Decreased pup body weights occurred at 1.25 and 2.5 mg/kg in rats, when exposed during the perinatal period (late gestation, parturition and lactation). At 5 mg/kg and higher doses (1.2 times the MRHD), prolonged gestation, dystocia and reduced maternal care were produced with corresponding increases in late fetal deaths and stillbirths and decreased birth weight, live litter size and pup survival. Insufficient numbers of pups survived at 5 mg/kg to evaluate the offspring for reproductive performance.


In studies in which pregnant rats were given nebivolol during organogenesis, reduced fetal body weights were observed at maternally toxic doses of 20 and 40 mg/kg/day (5 and 10 times the MRHD), and small reversible delays in sternal and thoracic ossification associated with the reduced fetal body weights and a small increase in resorption occurred at 40 mg/kg/day (10 times the MRHD). No adverse effects on embryo-fetal viability, sex, weight or morphology were observed in studies in which nebivolol was given to pregnant rabbits at doses as high as 20 mg/kg/day (10 times the MRHD).



Labor and Delivery


Nebivolol caused prolonged gestation and dystocia at doses ≥5 mg/kg in rats (1.2 times the MRHD). These effects were associated with increased fetal deaths and stillborn pups, and decreased birth weight, live litter size and pup survival rate, events that occurred only when nebivolol was given during the perinatal period (late gestation, parturition and lactation).


No studies of nebivolol were conducted in pregnant women. Bystolic should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.



Nursing Mothers


Studies in rats have shown that nebivolol or its metabolites cross the placental barrier and are excreted in breast milk. It is not known whether this drug is excreted in human milk.


Because of the potential for β-blockers to produce serious adverse reactions in nursing infants, especially bradycardia, Bystolic is not recommended during nursing.



Geriatric Use


Of the 2800 patients in the U.S. sponsored placebo-controlled clinical hypertension studies, 478 patients were 65 years of age or older. No overall differences in efficacy or in the incidence of adverse events were observed between older and younger patients.



Pediatric Use


Safety and effectiveness in pediatric patients have not been established. Pediatric studies in ages newborn to 18 years old have not been conducted because of incomplete characterization of developmental toxicity and possible adverse effects on long-term fertility (see Carcinogenesis, Mutagenesis and Impairment of Infertility).



Adverse Reactions


The data described below reflect worldwide clinical trial exposure to Bystolic in 6545 patients, including 5038 patients treated for hypertension and the remaining 1507 subjects treated for other cardiovascular diseases. Doses ranged from 0.5 mg to 40 mg. Patients received Bystolic for up to 24 months, with over 1900 patients treated for at least 6 months, and approximately 1300 patients for more than one year. In placebo-controlled clinical trials comparing Bystolic with placebo, discontinuation of therapy due to adverse events was reported in 2.8% of patients treated with nebivolol and 2.2% of patients given placebo. The most common adverse events that led to discontinuation of Bystolic were headache (0.4%), nausea (0.2%) and bradycardia (0.2%).



Adverse Reactions in Controlled Trials


Table 2 lists treatment-emergent signs and symptoms that were reported in three 12-week, placebo-controlled monotherapy trials involving 1597 hypertensive patients treated with either 5 mg, 10 mg or 20-40 mg of Bystolic and 205 patients given placebo and for which the rate of occurrence was at least 1% of patients treated with nebivolol and greater than the rate for those treated with placebo in at least one dose group.

Other Adverse Events Observed During Worldwide Clinical Trials


Listed below are other reported adverse events with an incidence of at least 1% in the more than 5300 patients treated with Bystolic in controlled or open-label trials, whether or not attributed to treatment, except for those already appearing in Table 2, terms too general to be informative, minor symptoms, or events unlikely to be attributable to drug because they are common in the population. These adverse events were in most cases observed at a similar frequency in placebo-treated patients in the controlled studies.

Metabolic and Nutritional Disorders: hypercholesterolemia and hyperuricemia


Nervous System Disorders: paraesthesia



Laboratory


In controlled monotherapy trials, Bystolic was associated with an increase in BUN, uric acid, triglycerides and a decrease in HDL cholesterol and platelet count.



Events Identified from Spontaneous Reports of Bystolic Received Worldwide


The following adverse events have been identified from spontaneous reports of Bystolic received worldwide and have not been listed elsewhere. These adverse events have been chosen for inclusion due to a combination of seriousness, frequency of reporting or potential causal connection to Bystolic. Events common in the population have generally been omitted. Because these events were reported voluntarily from a population of uncertain size, it is not possible to estimate their frequency or establish a causal relationship to Bystolic exposure: abnormal hepatic function (including increased AST, ALT and bilirubin), acute pulmonary edema, acute renal failure, atrioventricular block (both second and third degree), bronchospasm, erectile dysfunction, hypersensitivity (including urticaria, allergic vasculitis and rare reports of angioedema), myocardial infarction, pruritus, psoriasis, Raynaud's phenomenon, peripheral ischemia/claudication, somnolence, syncope, thrombocytopenia, various rashes and skin disorders, vertigo, and vomiting.



Overdosage


In clinical trials and worldwide postmarketing experience there were reports of Bystolic overdose. The most common signs and symptoms associated with Bystolic overdosage are bradycardia and hypotension. Other important adverse events reported with Bystolic overdose include cardiac failure, dizziness, hypoglycemia, fatigue and vomiting. Other adverse events associated with β-blocker overdose include bronchospasm and heart block.


The largest known ingestion of Bystolic worldwide involved a patient who ingested up to 500 mg of Bystolic along with several 100 mg tablets of acetylsalicylic acid in a suicide attempt. The patient experienced hyperhydrosis, pallor, depressed level of consciousness, hypokinesia, hypotension, sinus bradycardia, hypoglycemia, hypokalemia, respiratory failure and vomiting. The patient recovered.


Due to extensive drug binding to plasma proteins, hemodialysis is not expected to enhance nebivolol clearance.


If overdose occurs, Bystolic should be stopped and general supportive and specific symptomatic treatment should be provided. Based on expected pharmacologic actions and recommendations for other β-blockers, the following general measures should be considered when clinically warranted:



Bradycardia: Administer IV atropine. If the response is inadequate, isoproterenol or another agent with positive chronotropic properties may be given cautiously. Under some circumstances, transthoracic or transvenous pacemaker placement may be necessary.



Hypotension: Administer IV fluids and vasopressors. Intravenous glucagon may be useful.



Heart Block (second or third degree): Patients should be carefully monitored and treated with isoproterenol infusion. Under some circumstances, transthoracic or transvenous pacemaker placement may be necessary.



Congestive Heart Failure: Initiate therapy with digitalis glycoside and diuretics. In certain cases, consideration should be given to the use of inotropic and vasodilating agents.



Bronchospasm: Administer bronchodilator therapy such as a short acting inhaled β2-agonist and/or aminophylline.



Hypoglycemia: Administer IV glucose. Repeated doses of IV glucose or possibly glucagon may be required.


In the event of intoxication where there are symptoms of shock, treatment must be continued for a sufficiently long period consistent with the 12-19 hour effective half-life of Bystolic. Supportive measures should continue until clinical stability is achieved.

Bystolic Dosage and Administration


The dose of Bystolic should be individualized to the needs of the patient. For most patients, the recommended starting dose is 5 mg once daily, with or without food, as monotherapy or in combination with other agents. For patients requiring further reduction in blood pressure, the dose can be increased at 2-week intervals up to 40 mg. A more frequent dosing regimen is unlikely to be beneficial.



Renal Impairment


In patients with severe renal impairment (ClCr less than 30 mL/min) the recommended initial dose is 2.5 mg once daily; upward titration should be performed cautiously if needed. Bystolic has not been studied in patients receiving dialysis (see CLINICAL PHARMACOLOGY, Special Populations).



Hepatic Impairment


In patients with moderate hepatic impairment, the recommended initial dose is 2.5 mg once daily; upward titration should be performed cautiously if needed. Bystolic has not been studied in patients with severe hepatic impairment and therefore it is not recommended in that population (see PRECAUTIONS and CLINICAL PHARMACOLOGY, Special Populations).



Geriatric Patients


It is not necessary to adjust the dose in the elderly (see above and PRECAUTIONS, Geriatric Use).



CYP2D6 Polymorphism


(see CLINICAL PHARMACOLOGY, Pharmacokinetics)


No dose adjustments are necessary for patients who are CYP2D6 poor metabolizers. The clinical effect and safety profile observed in poor metabolizers were similar to those of extensive metabolizers.



How is Bystolic Supplied


Bystolic is available as tablets for oral administration containing nebivolol hydrochloride equivalent to 2.5, 5, 10, and 20 mg of nebivolol.


Bystolic tablets are triangular-shaped, biconvex, unscored, differentiated by color and are engraved with “FL” on one side and the number of mg (2 ½ , 5, 10, or 20) on the other side.
